A certificate of insurance, in full compliance with Section 95.52 (A)(14)(b) of the Tinley Park Village code [Section 714 (F)(11) of the Comprehensive Building Code] must accompany this application.
A $1,000 cash bond, as required by Section 95.52 (A)(15) of the Tinley Park Village Code [Section 714(F)(12) of the Comprehensive Building Code] must accompany this Application along with a $500 non-refundable Permit fee.
By filing this application, the Fireworks Company and Sponsoring Organization represent and warrant that they have reviewed, and are familiar with, the requirements for a Supervised Public Display of Fireworks as set forth in Section 95.52 of the Tinley Park Village Code and Section 714 of the Comprehensive Building Code, and hereby agree to comply with said requirements.
In consideration for the issuance of the Supervised Public Display of Fireworks Permit being applied for pursuant to this application, the Sponsoring Organization and Fireworks Company (hereinafter collectively referred to as the APPLICANTS) hereby agree to indemnify and save harmless the Village of Tinley Park (hereinafter the VILLAGE) and its officers, agents, employees, representatives and assigns, from lawsuits, actions, costs (including attorneys’ fees), claims or liabilities of any character brought because of any injuries or damages received or sustained by any person, persons, or property on account of any act or omission, neglect or misconduct of said APPLICANT, their respective officers, agents and/or employees arising out of, or in performance of the fireworks display referenced in this Application; or from any claims or amounts arising or recovered under any law, ordinance, order or decree. In connection with any such claims, lawsuits, actions or liabilities, the VILLAGE and its officers, agents, employees, representatives and their assigns shall each have the right to defense, counsel of their choice. The APPLICANTS shall be solely liable for all costs of such defense and for all expenses, fees, judgments, settlements and all other costs arising out of such claims, lawsuits, action or liabilities.
