Senior Program Field Trip Waiver and Release
Please Read Carefully Before Signing
This form explains your rights and responsibilities when participating in a Village-sponsored field trip or senior program. By signing below, you acknowledge that you have read, understand and agree to the terms outlined in this document.
1. Program Description and Participation
I understand that I am voluntarily registering for and participating in a Village of Tinley Park senior program and/or field trip (“Program”). This may include local or out-of-state travel, transportation by bus or other means, walking tours, group activities, and other related events.
I agree to follow all instructions provided by Village staff and to behave in a safe and respectful manner during the Program.
2. Acknowledgment and Assumption of Risk
I understand that participation in the Program involves certain risks. These may include, but are not limited to:
- Slips, trips, or falls
- Injuries during walking or recreational activities
- Risks related to transportation (including bus or air travel)
- Weather-related conditions
- Delays, changes in schedule, or other unforeseen events
I voluntarily accept and assume all risks, known and unknown, associated with my participation, including the risk of injury, illness, death, property damage, or loss.
3. Release of Liability
In consideration for being allowed to participate in the Program, I agree to release and discharge the Village of Tinley Park and its officials, officers, employees, agents, and volunteers (“the Village”) from any and all claims, demands, or causes of action that I may have now or in the future arising out of or related to my participation in the Program. This includes claims for personal injury, illness, death, property damage, or loss, even if caused by the negligence of the Village, to the fullest extent permitted by Illinois law.
4. Indemnification and Hold Harmless Agreement
I agree to indemnify, defend, and hold harmless the Village from any and all claims, damages, liabilities, costs, or expenses (including attorney’s fees) arising out of or related to my participation in the Program.
5. Transportation and Supervision
I understand that transportation may be provided as part of the Program. While reasonable care will be taken, I acknowledge that the Village does not guarantee my safety during transportation or at any Program location. I understand that Village staff and volunteers provide general supervision but cannot provide one-on-one care or continuous personal assistance.
6. Medical Information and Responsibility
I understand that:
- I am responsible for managing my own health and medical needs during the Program
- The Village does not provide medical care or administer medications
- In the event of an emergency, staff may call emergency services on my behalf
I authorize emergency medical treatment if I am unable to provide consent at the time.
8. Photo and Media Release
I give permission for the Village of Tinley Park to take photographs, video, or audio recordings of me during Program activities and to use these materials for promotional or informational purposes without compensation.
9. Refund Policy
I acknowledge that I have received, read, and understand the Program’s refund policy as provided in the registration materials.
